Living with an autoimmune condition can sometimes feel unpredictable. One week, symptoms may be relatively manageable. The next, fatigue, digestive discomfort, skin changes, headaches, joint pain or brain fog may suddenly become more noticeable. This uncertainty often leads people to ask an important question:

Could something I am eating be contributing to my symptoms?

Nutrition can play a valuable supportive role in autoimmune health. Food provides the protein, vitamins, minerals, essential fats and plant compounds the body needs to maintain tissues, support energy production and regulate normal immune function. Certain dietary patterns may also support cardiovascular, digestive and metabolic health, which can be especially important for people living with chronic inflammatory conditions.

However, nutrition should not be presented as a cure for autoimmune disease. Autoimmune conditions occur when the immune system mistakenly targets the body’s own cells or tissues. More than 80 autoimmune diseases have been identified, and they can affect nearly every part of the body. Examples include rheumatoid arthritis, lupus, multiple sclerosis, type 1 diabetes, celiac disease, Hashimoto’s thyroiditis, psoriasis and inflammatory bowel disease.

Different autoimmune conditions behave differently. A food that aggravates digestive symptoms in one person may cause no difficulty for another. This is why a personalized, evidence-informed approach is generally more useful than following a long list of foods labelled “inflammatory.”

For some people, a carefully planned elimination diet can help identify individual food triggers. The key is to approach it as a temporary and structured investigation, followed by reintroduction not as a permanently restrictive lifestyle.

How Nutrition Supports Autoimmune Health

Food does not simply provide calories. Nutrients obtained through food are involved in immune regulation, tissue repair, hormone production, bone health, energy metabolism and the maintenance of the intestinal barrier.

People with autoimmune conditions may also have nutritional needs related to:

  • Reduced appetite during symptom flares

  • Digestive symptoms or poor nutrient absorption

  • Dietary restrictions

  • Chronic inflammation

  • Medication side effects

  • An increased risk of osteoporosis

  • Fatigue that makes cooking difficult

  • Coexisting conditions such as anemia, diabetes or cardiovascular disease

For example, corticosteroid medications may affect blood glucose and bone health. Certain digestive autoimmune diseases can interfere with the absorption of iron, vitamin B12, folate, calcium or vitamin D. Some medications must also be taken with food, while others may interact with alcohol, grapefruit or particular supplements. Nutrition therefore needs to support the whole person not simply remove foods.

Begin With an Anti-Inflammatory Foundation

Before removing several food groups, it is often more helpful to improve the overall quality of the diet. A Mediterranean-style eating pattern is one example of a balanced approach. It emphasizes vegetables, fruit, whole grains, beans, lentils, nuts, seeds, olive oil and fish, with moderate amounts of other protein and dairy foods. It generally limits heavily processed foods, refined carbohydrates, excess sodium, added sugars and large amounts of saturated fat.

The American College of Rheumatology conditionally recommends a Mediterranean-style eating pattern for people with rheumatoid arthritis. Importantly, it does not recommend highly restrictive, formally defined diets as a replacement for appropriate medical treatment. A balanced anti-inflammatory foundation may include:

Plenty of colourful vegetables and fruit

Vegetables and fruit provide fibre, vitamins, minerals and a wide variety of plant compounds. Try to include different colours across the week rather than relying on one supposed “superfood.” Options may include:

  • Leafy greens

  • Broccoli

  • Cauliflower

  • Carrots

  • Peppers

  • Tomatoes

  • Squash

  • Berries

  • Apples

  • Citrus fruit

  • Pears

  • Pomegranate

Frozen produce is just as practical when fresh food is unavailable or difficult to prepare.

Adequate protein

Protein supports muscle tissue, immune proteins, enzymes, hormones and recovery. This may be especially important when chronic illness reduces appetite or physical activity. Protein options include:

  • Fish and seafood

  • Eggs

  • Chicken or turkey

  • Tofu or tempeh

  • Beans and lentils

  • Greek yogurt

  • Cottage cheese

  • Lean meat

  • Nuts and seeds

The appropriate sources will depend on a person’s preferences, tolerances and any foods being tested during an elimination diet.

Fibre-rich carbohydrates

Carbohydrates do not automatically cause inflammation. Whole grains, beans, lentils, vegetables and fruit can provide valuable fibre and nutrients. Examples include:

  • Oats

  • Barley

  • Quinoa

  • Brown or wild rice

  • Whole-grain bread

  • Beans

  • Chickpeas

  • Lentils

  • Potatoes and sweet potatoes

  • Fruit

Certain medical conditions may temporarily require a different fibre intake. For example, a person experiencing an inflammatory bowel disease flare, intestinal narrowing or severe diarrhea may need individualized guidance rather than being told simply to “eat more fibre.”

Unsaturated fats

Unsaturated fats can be obtained from:

  • Olive oil

  • Avocado

  • Nuts

  • Seeds

  • Natural nut and seed butters

  • Fatty fish

These foods can support cardiovascular health and make meals more satisfying.

What Is an Elimination Diet?

An elimination diet is a temporary eating plan in which a specific food or group of foods is removed and later reintroduced while symptoms are monitored. The purpose is to answer a focused question:

Does this particular food reliably make my symptoms worse?

The removal phase by itself cannot provide a complete answer. Symptoms may improve for many unrelated reasons, including changes in stress, sleep, medication, menstrual cycles, infections, physical activity or the natural pattern of the condition.

The reintroduction phase is therefore essential. If symptoms repeatedly return after the same food is reintroduced, that provides more useful information than feeling better during a period when many foods were removed simultaneously. Elimination diets are typically based on removing selected foods for a limited period and then reintroducing them while monitoring symptoms.

When Can an Elimination Diet Be Beneficial?

A structured elimination diet may be helpful when:

  • Symptoms appear repeatedly after a particular food

  • A healthcare professional suspects a food allergy or intolerance

  • Digestive symptoms coexist with an autoimmune condition

  • A diagnosed gastrointestinal condition has an evidence-based dietary protocol

  • A person wants to distinguish a genuine trigger from a food they have been unnecessarily avoiding

  • A dietitian or clinician is supervising the investigation

An elimination diet may help identify foods that worsen symptoms such as:

  • Bloating

  • Abdominal pain

  • Diarrhea

  • Reflux

  • Headaches

  • Fatigue

  • Skin symptoms

  • Joint discomfort

However, these symptoms are not specific to food reactions. They may also be caused by the autoimmune condition itself, medication side effects, anemia, thyroid dysfunction, infection, sleep difficulties or another health problem. An elimination diet should not delay appropriate medical investigation.

Elimination Diets Are Not the Same as Treating Every Autoimmune Disease With Food

It is important to distinguish between a disease with a known dietary treatment and an autoimmune condition in which food may only influence certain symptoms.

Celiac disease

Celiac disease is an autoimmune condition in which gluten causes an immune reaction that damages the small intestine. Once celiac disease has been properly diagnosed, strict lifelong removal of gluten is the established treatment.

This is different from removing gluten simply because a person has Hashimoto’s thyroiditis, rheumatoid arthritis or another autoimmune diagnosis. Gluten has not been established as a universal trigger for all autoimmune conditions.

Anyone who may have celiac disease should be tested before beginning a gluten-free diet. Avoiding gluten before testing can produce inaccurate results and make the diagnostic process more difficult.

Diagnosed food allergy

For a confirmed food allergy, avoiding the allergen may be medically necessary. A food allergy can involve rapid symptoms such as hives, swelling, vomiting, wheezing, difficulty breathing or anaphylaxis.

Anyone with a history of an immediate or serious reaction should not deliberately reintroduce the food at home. Oral food challenges for suspected allergies should be planned by an allergist in an appropriate medical setting.

Food intolerance

Food intolerance does not necessarily involve an autoimmune or allergic immune response. Lactose intolerance, for example, occurs when a person has difficulty digesting lactose.

The amount consumed may matter. Some people can tolerate a small quantity but develop symptoms after a larger portion. The goal may therefore be identifying a personal threshold rather than permanently removing the food.

Symptom-triggering foods

A food may worsen bloating, reflux, headaches or another symptom without causing an allergy or autoimmune reaction. A structured elimination and reintroduction process can sometimes help identify these patterns.

What About the Autoimmune Protocol Diet?

The Autoimmune Protocol, commonly called the AIP diet, is a broad elimination approach that may remove grains, legumes, dairy, eggs, nuts, seeds, nightshade vegetables, alcohol, coffee, food additives and several other foods.

Some small studies have reported improvements in symptoms or quality of life among selected people with inflammatory bowel disease. However, early studies have often been small, uncontrolled or limited to particular patient populations. This means they cannot establish that the AIP diet treats autoimmune disease generally or that such a restrictive diet is necessary for everyone.

The more foods a diet removes, the more difficult it becomes to determine which food if any was responsible for a change in symptoms. Highly restrictive diets can also increase the risk of inadequate fibre, calcium, iron and other nutrients.

A full AIP diet should therefore not automatically be the first step. A targeted approach that tests one food or a small number of likely triggers may be easier, safer and more informative.

How to Do an Elimination Diet

Step 1: Speak With a Qualified Healthcare Professional

Before changing your diet, discuss your symptoms with your physician, nurse practitioner or another qualified clinician. A registered dietitian can help you maintain adequate nutrition during the process.

This step is particularly important if you:

  • Are pregnant or breastfeeding

  • Are a child or adolescent

  • Have diabetes

  • Have kidney or liver disease

  • Have inflammatory bowel disease

  • Are underweight or unintentionally losing weight

  • Have anemia or nutrient deficiencies

  • Have a history of an eating disorder

  • Have difficulty swallowing

  • Have experienced anaphylaxis

  • Are removing several major food groups

  • Take medications that may need adjustment with dietary changes

An elimination diet should not replace prescribed medication. Do not reduce or discontinue medication simply because symptoms temporarily improve.

Step 2: Rule Out Conditions That Require Testing

Some foods should not be removed before appropriate medical testing. The most important example is gluten. If symptoms or family history suggest possible celiac disease, testing should occur while gluten is still being consumed. Beginning a gluten-free diet first may interfere with the accuracy of celiac blood tests and biopsies. Other symptoms may require testing for:

  • Food allergy

  • Lactose intolerance

  • Anemia

  • Thyroid dysfunction

  • Infection

  • Inflammatory bowel disease

  • Medication side effects

  • Other digestive conditions

It is better to investigate these possibilities than to assume that every symptom is a food sensitivity.

Step 3: Choose Specific Symptoms to Track

Decide what you are trying to improve before beginning. Rather than tracking a vague feeling such as “inflammation,” choose observable symptoms, including:

  • Number of bowel movements

  • Stool consistency

  • Abdominal pain from zero to ten

  • Bloating

  • Headache frequency

  • Morning joint stiffness

  • Skin itching

  • Fatigue

  • Sleep quality

  • Reflux

  • Medication use

Try to track symptoms at approximately the same time each day. This creates a clearer baseline and reduces the tendency to judge the diet based on one unusually good or bad day.

Step 4: Keep a Baseline Food and Symptom Diary

For one to two weeks, record:

  • Foods and drinks

  • Approximate portions

  • Meal times

  • Symptoms and their timing

  • Sleep

  • Stress

  • Exercise

  • Menstrual cycle changes

  • Medications and supplements

  • Any illnesses or infections

This may reveal that symptoms follow a specific food, a portion size, skipped meals, alcohol, lack of sleep or another factor. A written diary is generally more reliable than trying to remember what was eaten several days earlier.

Step 5: Select the Foods Carefully

Avoid beginning with a long, generic list from social media. Choose one food or a small number of foods based on:

  • A repeated symptom pattern

  • Medical history

  • A clinician’s recommendation

  • A diagnosed condition

  • Foods consumed frequently enough to evaluate

Depending on the situation, foods tested might include:

  • Cow’s milk

  • Wheat

  • Eggs

  • Soy

  • A particular food additive

  • A specific high-FODMAP food

  • Another individually suspected item

Removing six or more major food groups is rarely necessary as a first step. The commonly discussed six-food elimination diet was designed primarily for conditions such as eosinophilic esophagitis not as a general diet for every autoimmune condition.

Step 6: Plan Nutritious Replacements

Do not simply remove a food. Replace the nutrients and functions it provided.

When removing dairy

Possible replacements include:

  • Fortified soy beverage

  • Calcium-set tofu

  • Canned salmon or sardines with bones

  • Fortified dairy-free yogurt

  • Beans

  • Tahini

  • Leafy greens

Check whether dairy alternatives contain adequate calcium, vitamin D and protein. Many almond, oat or coconut beverages contain substantially less protein than cow’s milk or fortified soy beverages.

When removing wheat

Possible alternatives include:

  • Rice

  • Quinoa

  • Buckwheat

  • Certified gluten-free oats

  • Potatoes

  • Corn

  • Millet

  • Legumes

Remember that wheat-free and gluten-free do not mean the same thing. Barley and rye contain gluten even though they are not wheat.

When removing eggs

Possible protein alternatives include:

  • Tofu

  • Beans

  • Lentils

  • Fish

  • Poultry

  • Meat

  • Greek-style dairy or fortified alternatives

Egg replacements for baking may include ground flaxseed, chia seeds or commercial egg replacers, depending on the recipe.

Step 7: Complete the Elimination Phase

During this phase, remove the selected food consistently while continuing to eat a varied and nutritionally balanced diet. Many structured elimination protocols last approximately two to six weeks, although the appropriate duration depends on the suspected reaction and the medical condition. Some clinical elimination diets use a four-to-six-week removal phase followed by gradual reintroduction.

During the elimination phase:

  • Read ingredient labels

  • Check sauces, seasonings and packaged foods

  • Plan meals before eating out

  • Continue recording symptoms

  • Keep medications stable unless your prescriber changes them

  • Avoid making several unrelated lifestyle changes at the same time

  • Eat enough food

The goal is not to eat as little as possible. A successful elimination diet remains satisfying and nutritionally complete.

If there is no meaningful symptom improvement, the removed food may not be a trigger. This is useful information and may prevent years of unnecessary restriction.

Step 8: Reintroduce One Food at a Time

Reintroduction is the most important step. Once symptoms are reasonably stable, reintroduce one eliminated food at a time. Choose a simple version of the food so that the result is easier to interpret. For a suspected non-allergic food trigger, a clinician may recommend:

  1. Eating a small amount of the food

  2. Increasing to a normal serving if no concerning symptoms occur

  3. Monitoring symptoms over the following days

  4. Returning to the elimination baseline before testing the next food

The exact schedule should be personalized. Some symptoms occur within hours, while others may take longer to assess. Do not introduce multiple foods at once. If symptoms occur, you will not know which food was responsible.

Do not perform a home reintroduction if the food has ever caused difficulty breathing, throat tightness, facial swelling, widespread hives, fainting or another possible allergic reaction. Seek guidance from an allergist.

Step 9: Confirm the Pattern

A single episode does not always establish a trigger. Consider:

  • Did the same symptom happen more than once?

  • Was the reaction reasonably close to eating the food?

  • Could stress, sleep, infection or medication explain it?

  • Does the symptom improve again when the food is removed?

  • Was the portion unusually large?

  • Is the reaction caused by the food itself or something commonly eaten with it?

A reproducible pattern is more meaningful than one isolated reaction.

Step 10: Build a Personalized Long-Term Diet

The final goal is to eat the widest variety of foods that can be comfortably and safely tolerated. Foods that do not cause symptoms should be returned to the diet. Keeping unnecessary restrictions provides no additional benefit and may reduce nutritional adequacy, enjoyment, flexibility and quality of life. If a food appears to be a genuine trigger, the long-term plan may involve:

  • Avoiding it

  • Eating a smaller portion

  • Consuming it less frequently

  • Choosing a different preparation method

  • Using an appropriate enzyme, such as lactase

  • Treating an underlying digestive condition

  • Reassessing tolerance later with professional guidance

The outcome should be a personalized diet—not a permanent elimination phase.

Be Careful With Food-Sensitivity Testing

Commercial food-sensitivity panels often measure food-specific IgG antibodies. These tests are commonly marketed as a way to identify foods that cause fatigue, inflammation, digestive symptoms or autoimmune flares.

However, food-specific IgG frequently reflects exposure to a food and may indicate tolerance rather than disease. The Canadian Society of Allergy and Clinical Immunology and other allergy organizations recommend against using IgG testing to diagnose food allergies or food intolerances. These panels may produce long lists of “positive” foods and encourage people to remove many nutritious items unnecessarily.

A symptom history, appropriate medical testing and a structured elimination-and-reintroduction process are more informative than choosing foods based solely on an IgG panel.

What Are the Risks of an Elimination Diet?

An elimination diet may become harmful when it is too broad, lasts too long or is poorly planned. Potential risks include:

  • Inadequate calorie or protein intake

  • Iron deficiency

  • Low calcium or vitamin D intake

  • Reduced fibre intake

  • Vitamin B12 or folate deficiency

  • Constipation

  • Unintentional weight loss

  • Increased food anxiety

  • Social isolation

  • Greater meal-preparation costs

  • Disordered eating patterns

  • Poor growth in children

The risk generally increases as more foods are removed. Children and people who already have limited diets are particularly vulnerable to nutritional deficiencies.

Stop and seek professional support if the diet causes dizziness, weakness, rapid weight loss, worsening fatigue, fear of eating or an increasingly narrow list of “safe” foods.

The Bottom Line

Nutrition is an important part of supporting overall health when living with an autoimmune condition. A nutrient-dense eating pattern can support energy, bone health, muscle maintenance, digestive function and cardiovascular health. For certain people, a carefully selected elimination diet can be a valuable tool for identifying foods that consistently worsen symptoms. It is most useful when it is:

  • Targeted

  • Temporary

  • Properly planned

  • Nutritionally complete

  • Followed by systematic reintroduction

An elimination diet is not a detox, a cleanse or a universal cure for autoimmune disease. More restriction does not necessarily produce more healing.

The purpose is not to create fear around food. It is to gather useful information, identify genuine triggers and ultimately build the most varied, nourishing and enjoyable diet possible.
